Proposed New Mission

Referring to health care specialists when there is no need for referral.
Program Goal Statement
To educate and empower family members and community health care workers in the identification and management of symptoms for chronically/ terminally ill patients within one year (1) for five rural communities within the catchment area of Komfo Anokye Teaching Hospital.
Long Term Outcome Smart Objective
To reduce the Specialists clinical review frequency of the chronically/ terminally ill patients from focus rural areas to 40% by October 31, 2017.
Measure
To organize two (2) paired training sessions within each focus community for twenty family members and ten community health workers in the identification and management of symptoms of chronically/terminally ill patients by the end of October, 2017.
