Olembe District Hospital: Who We Are 

• Olembe District Hospital is a level 3 health service center in Cameroon. It is situated in the outskirts of Yaounde, capital of Cameroon.
• It provides comprehensive clinical and basic paraclinical health services.
• We have 4 main departments and several concurrent activities.
• Diabetes and Hypertension clinic where clients are received every Thursday (30clients/ session). 

Mission Statement 

Provide  comprehensive medical care to the population of Yaounde by offering up to date clinical, paraclinical and community services.

Summary of Project

• Incidence of CV complications has risen from 10-20% in patients 40years and above who are currently followed up for Hypertension and  Diabetes  (stroke, cardiopathies, retinopathies,nephropathies, neuropathies).
• Main Cause: Poor understanding of their pathology and its complications by the patients. 
• Solution: Health Education
• Critical Issues the Project Addresses (focus area): Improvement of health outcomes
• How does your organization fit in? We run a diabetes/ HBP clinic in the outskirts of the town away from the major hospitals.
• Why is this project an opportunity for you? A sick population does not contribute to the economy of the nation.

Goal of the CHIP

To reduce  morbidity and mortality in patients aged 40yrs and above who are followed up for hypertension and diabetes in Olembe District Hospital (ODH).

Objectives

Process Objective:

•  Obtain the necessary teaching aids by end of   December 2013. 
•  Successfully carry out first training session in March 2014. (40 patients, pre/post test scores). 

Outcome Objective:

• Reduce incidence of  CV complications  from 20% to 8% in patients 40yrs and above who are  currently followed up by end of Dec 2014.

Action Plan

• Acquire necessary material used to diagnose disease and complications ( Sept-Oct 2013).
• Recycle personnel involved(2 weeks-Oct 2013).
• Acquire the teaching aids  (Nov 2013).
• Train peer educators (Nov 2013).
• 1st Training session (Dec 2013)

Monitoring & Evaluation Plan

Indicators  (baseline, frequently)

• Number of flip charts, pamphlets, projectors, peer educators available (none, weekly)
• Percentage increase in post training scores compared to pre training scores ( at each session)
• Number of patients being followed up who develop complications (monthly)

Reducing incidence of cardiovascular complications in hypertensive/diabetic  patients
