About the Organization
The Asokore Mampong Municipality is one of the newly created municipalities in the Ashanti Region of Ghana by Legislative Instrument (L.I) 2112, started its operations in 2012. The Health Directorate came into the Municipality in 2013 to start its health care deliveries. In the year 2013 the municipality had a population of 280,707. The current projected population (2015) is 348,273, (PHC 2010). There are 6 sub municipals with 12 private and 2 public health facilities.
Mission
To contribute to socio-economic development and the development of a local health industry by promoting health and vitality through access to quality health for all people living in the Asokore Mampong Municipality, using motivated personnel
External Analysis
Low immunization coverage among children under 1yr (30% in 2014 , with PENTA 3 as proxy).
This could result in the outbreak of vaccine preventable diseases as the missed children increase cumulatively over time

Solution: Increasing immunization coverage among children under 1yr from 30% to 50% by June 2016 through awareness creation and effective involvement of private facilities
Rationale: Increasing immunization coverage will improve child health and reduce any possible vaccine preventable disease outbreak
Goals and Objectives
Goal: Increase immunization coverage among children under 1yr from 30% to 50% by June 2016, and 50%-90% by 2019
Long term objective: Increase immunization coverage among children under 1yr from 30% 90% by 2019
Short term objective 1: create awareness on importance of immunization in 90% of communities by June 2016
Short term objective 2: To sensitize all private health facilities monthly on the importance of immunization by June 2016

- Monthly review of EPI report against targets

- Annual performance review
Lessons Learnt
- With careful planning, a lot can be achieved with little resources
- MDI-CHIP project has the potential of improving the lives of most vulnerable in the communities
- The experienced gained will help us mobilize resources for effective health care delivery.
