To establish partnerships between Hygeia Community healthcare and other Stakeholders in targeted communities to improve process efficiency.

CHIP Outcome Objectives

  • To maintain medical information from within the past 3 months on 70% of Hygeia HIV patients by the end of June 2014.
  • Work out collaboration mechanisms to refer and receive feedback on Hygeia HIV patients  referred to ART sites funded by parallel programs.
