CHIP Project Selection

Summary of Project: 

Pogbi maternity offers services to prevent mother-to child HIV transmission. However, all HIV positive women do not end the process of prevention because of lost to follow up, difficulties in getting referral reports on women with HIV for assessment, the difficulties screening spouses and sometimes breaks inputs for HIV. 

Critical Issues the Project Addresses: 

Finding lost to follow up

  • Follow  up on the  referral reports for the women with HIV to help improve their health condition. 
  • Avoid the breakage in screening inputs
  • To promote HIV testing  of spouses.
  • Health promotion among pregnant women

Goal

  • To ensure that the number of pregnant women tested positive for HIV at Pogbi Maternity go through comprehensive PMTCT Care

Process Objective

  • To create awareness among 100% of pregnant women at the ANC Clinic in Pobi Maternity by 2013

Outcome Objective

  • To increase the number of pregnant women going through PMTCT and comprehensive HIV care from  40% to  60% by Decembr 2014
