It is the second largest tertiary hospital in Ghana. The hospital has about 1,200 bed capacity with average annual outpatients flow of over 450,000. It has a current staff population of about 4,000. Services

To provide quality services to meet the needs and expectations of its clients. This will be achieved through well motivated and committed staff applying best practices and innovation.

Proposed New Mission

To train family members and community health care workers to identify and manage the symptoms of chronically or terminally ill patients in peri-urban and rural areas in the Ashanti region to reduce unwarranted clinical reviews without compromising on the quality of care of patients.

Current State Desired Future State
1. Inadequate health care professionals in managing the chronically/ terminally ill patients in the communities. 1. Empower family members, community volunteers and community health workers with the requisite knowledge and skills to assess and manage symptoms of the chronically/ terminally ill patients.
2. Inadequate/ Lack of teaching materials to inform family and community members for effective early detection and management of symptoms of chronically/ terminally ill patients in the communities. 2. Use of posters, flip-charts, film and role-play which is culturally accepted in relaying information.
3. Lack or limited access to specialized health care professionals in managing the chronically/ terminally ill patients. 3. Provide opportunities for phone reviews and consultations

 

Project Selection

Selected Problem Area: Referring to health care specialists when there is no need for referral.

Primary Focus Area: Quality of and access to care and treatment, Quality of and access to palliative, nutritional and psychosocial management and/or treatment of the chronically/ terminally ill patients.

Description of Solution (CHIP): Quality of and access to palliative, nutritional and psychosocial management and/or treatment of the chronically/ terminally ill patients.

Program Goal Statement

To educate and empower family members and community health care workers in the identification and management of symptoms for chronically/ terminally ill patients within one year (1) for five rural communities within the catchment area of Komfo Anokye Teaching Hospital.

SMART Objectives

To reduce the Specialists clinical review frequency of the chronically/ terminally ill patients from focus rural areas to 40% by October 31, 2017.
